In attending SWINGDYHOP dance classes you agree that your child is fit to participate in physical activity.

That the provider is made aware of any medical requirements (such as asthma, epilepsy or any under lying condition), in the health information on sign up.

That the provider is made aware of your decision over photo usage.

That you agree to collect and drop off your children at the designated times.
